  • have a written medical referral

  • are medically stable

  • are able to transfer with one person or independently

  • are able to learn new tasks

  • have the potential to improve functionally

  • are motivated and willing to participate

  • are a resident of Niagara with a valid health card

  • live in a community setting, including retirement homes

  • are able to arrange own transportation (see contact information)

HEALTH CARE PROFESSIONALS

  • Do you have a patient who is reporting decreasing ability to manage at home?  Do you think that if they could improve their mobility and function that they would be able to remain in their current living situation longer?

  • Do you have a patient who has been recently discharged from hospital?  Are they medically stable but deconditioned and eager to regain full function and mobility?
